At 43, a woman sits in her GP's office and names five things: brain fog in the meeting, new knee pain although she has not started extra sport, worse sleep despite the same routine, an LDL that has suddenly hit 165 mg/dL, and a belly that is growing even though the calories have not changed. What often goes unsaid: all five sit on the same hormone. She is not ill. She is in perimenopause.

In everyday language, estrogen is filed under reproduction. In the body it reaches into more than 400 biological processes, from how the brain uses glucose to how the liver handles cholesterol. Perimenopause starts at 47 on average and can begin up to ten years before the last period. Estradiol in this phase does not fall in a straight line; it swings from hour to hour. That is why women from 40 often meet a scatter of symptoms that no single reading explains and no single specialty owns. If you want to understand the transition, think of estrogen as a system regulator, not as a cycle marker.

Estrogen and the brain

The female brain uses around 20 percent more glucose than the male brain on average, and estrogen steers that energy metabolism. In 2017, Mosconi and colleagues used FDG-PET imaging in 43 clinically healthy women aged 40 to 60 and showed that cerebral glucose use already falls in perimenopause - in exactly the brain regions that later become Alzheimer-vulnerable (Mosconi et al., 2017 [1]). Mitochondrial activity in the brain cells fell in parallel. That explains part of the brain fog many women describe from their mid-forties, and it is one biological trail for why women carry two thirds of the Alzheimer's burden.

Estrogen, LDL and the heart

As long as estrogen is circulating, it keeps the LDL receptors in the liver active and cholesterol clearance steady. When the level falls, the blood picture changes in a measurable way. In 2019, Ambikairajah and team showed in a meta-analysis of 114,655 women from 66 studies that postmenopausal women, compared with premenopausal women, have on average higher triglycerides, higher total cholesterol and, above all, LDL cholesterol 0.45 mmol/L higher (Ambikairajah et al., 2019 [2]). HDL stays the same. In other words: the risk ratio shifts systematically in an unfavourable direction, and not because lifestyle suddenly failed, but because a hormone was lost. An LDL jump at 45 without a new diet mistake is therefore not a reason for self-blame. It is a signal that metabolism has flipped a switch. Anyone who wants to take this further looks at ApoB, the particle-number marker that maps the LDL reality more precisely than the standard LDL value.

Estrogen, insulin and belly fat

Estrogen keeps adipocytes where they are less dangerous: on the hips and thighs. When the level falls, fat moves into the abdomen and around the organs. That visceral fat is hormonally active, releases inflammatory messengers and pushes insulin resistance. That is why many women from 45 report that the old diet strategies no longer work. The calorie balance is not the main problem; the fat-distribution key is. {{callout}} That is what makes fasting insulin an interesting early indicator: it often shows change years before HbA1c or fasting glucose stand out.

Estrogen and bone

This is where the largest misunderstanding sits. Yes, estrogen slows bone resorption. But bone is not a hormone-only story; it is a mechanical and nutritional system. The bone matrix is around 30 percent collagen protein. With too little protein it becomes brittle, regardless of the estrogen level. Groenendijk and team showed in a 2019 meta-analysis of twelve cohort studies that protein intake above the recommended 0.8 g per kilogram lowers hip-fracture risk in older adults by 11 percent (hazard ratio 0.89) (Groenendijk et al., 2019 [3]). Watson and team showed in the 2018 LIFTMOR trial with 101 postmenopausal women with low bone density that eight months of high-intensity resistance training raised lumbar-spine bone density by 2.9 percent, while the comparison group lost 1.2 percent (Watson et al., 2018 [4]). Calcium plus vitamin D additionally lowers fracture risk by 15 percent, and hip-fracture risk by 30 percent (Weaver et al., 2016 [5]). Eating 40 prunes a day helps measurably too (De Souza et al., 2022 [6]). That is a supplementary lever, not the main strategy. The point worth keeping: four lifestyle levers (vitamin D, protein, strength training, creatine) act on bone through mechanics and nutrition. They do not replace medical care, and they do not require a single estradiol reading to start.

Estrogen, thyroid and sleep

Estrogen talks to the thyroid system on several levels. It raises the transport protein TBG, which binds fT4, and it influences the conversion of T4 to T3. In perimenopause, many women tip into a subclinical Hashimoto pattern that the standard TSH value misses. We unpacked that in Thyroid: why TSH alone misses 5 of 7 patterns. Anyone in perimenopause with new fatigue, weight change and feeling cold should want to see fT3, fT4 and Anti-TPO, not TSH alone. Sleep adds a second effect: estrogen stabilises deep-sleep architecture. When the level falls, sleep fragments, the cortisol axis compensates, and in the morning recovery no longer feels like recovery.

Why aescolab does not measure estradiol

Here is the honest answer to the obvious question: why would a blood-analysis company skip the very hormone this article is about? Three measurement reasons. First: estradiol swings hourly in perimenopause. A value at ten in the morning and one at three in the afternoon can differ by a factor of three, with nothing pathological behind it. Second: the value is only meaningful in its cycle phase, and that phase is often no longer clearly defined in perimenopause. Third: a single reading leads to false conclusions more often than to clarity - both in the direction of “everything is in range, so everything is fine” and in the direction of “value is low, so hormone deficiency”. The consequence: aescolab concentrates on the markers that still allow action in the hormonal transition. A blood test is not a diagnosis.

Concretely, for women between 40 and 55, that means this. Keep four markers in view: thyroid (TSH plus fT3 plus Anti-TPO), ferritin, vitamin D and the omega-3 index. Take four lifestyle levers seriously: enough protein (1.2 to 1.6 g per kilogram of body weight, spread across three meals), progressive strength training with real loads twice a week, vitamin D held year-round in the upper third of the reference range, and an omega-3 index moving toward 8 to 11 percent. For diffuse symptoms that do not fit a drawer, a systemic blood analysis helps more than an isolated hormone test. If the gynaecologist-GP ping-pong is going nowhere, and you know that everything is “in range” but something is off: that is exactly what a complete marker view is for. Understanding estrogen does not mean measuring estrogen. It means knowing the markers you can still act on.

Conclusion

Estrogen after 40 is a system regulator, not a single-hormone question. Anyone who reads the five symptoms - brain fog, new joint pain, worse sleep, an LDL rise, visceral belly fat - as one connected mechanism starts asking the right questions. A single estradiol reading does not answer those questions reliably. Four actionable markers (thyroid, ferritin, vitamin D, omega-3) come closer. They are still not a diagnosis.

Frequently asked questions

Should I have my estradiol measured?

If your gynaecologist needs a value in a concrete clinical context, yes. For a general “where do I stand” question after 40, a single reading gives too little information. A series would be better, but with hourly variation even that stays uncertain.

What helps with brain fog from 45?

Stabilise sleep architecture (regular times, a cool bedroom), check iron status and the thyroid, bring the omega-3 index up, strength-train twice a week. A clinically relevant vitamin D deficiency can make brain fog substantially worse. None of this replaces a medical work-up if symptoms are severe or new.

My LDL is suddenly high at 47, and nothing in my life has changed. What is going on?

The loss of estrogen reduces LDL-receptor activity in the liver. That is a systemic effect, not a personal failure. A sensible next step: measure ApoB, check visceral fat via waist-to-hip ratio, look at movement and protein intake. Fibre, including beta-glucans from oats and mushrooms, can help. Garlic, or a garlic-oil capsule, is a means that has held up in studies. If the values are pathological, medical advice is required.

When the gynaecologist, when the internist?

The gynaecologist for cycle changes, hot flushes, vaginal dryness, new mood swings. The internist for sudden blood-pressure, LDL or weight changes without another explanation. The blood analysis from aescolab is the systemic bridge between the two. It does not replace either specialist.
